Legacy Sperm Testing vs. Fellow Sperm Testing (Updated Oct 2026)

OVERALL: Legacy wins.

In comparing the Legacy at-home sperm test and the Fellow at-home sperm test, Legacy was the winner. As of October 2026, Legacy’s $145 Basic Semen Analysis is the lowest-priced lab-grade mail-in sperm test available, and Legacy is the only at-home option with a Sperm Score dashboard (Sperm Insights), sperm DNA fragmentation testing, and in-network health insurance coverage.

Fellow deserves credit for its science. Their validation study was published in Fertility and Sterility, they work with 4,000+ clinics, and their $199 kit is a solid, no-frills semen analysis. If your urologist hands you a Fellow box, you’re in good hands.

QUICK FACTS (Oct 2026):

  • Legacy’s cheapest sperm test is $145. Fellow’s is $199.
  • Legacy offers three testing tiers ($145, $295, $495). Fellow offers one ($199).
  • Legacy is the only at-home sperm test with DNA fragmentation testing.
  • Legacy is the only at-home sperm test with a Sperm Score and a lifetime results dashboard (Sperm Insights).
  • Both return results in about 48 hours after the lab receives your sample.
  • Legacy is in-network with Aetna, Cigna, Emblem and UnitedHealthcare. Fellow lists Progyny and Maven only.
  • Both labs are CLIA-certified. Legacy’s lab is also New York State (CLEP) approved for its full semen analysis.

IS LEGACY OR FELLOW CHEAPER FOR AN AT-HOME SPERM TEST?

Legacy is cheaper at the entry level. Legacy’s Basic Semen Analysis costs $145. Fellow’s only semen analysis costs $199.

Legacy now has three testing tiers:

  • Basic Semen Analysis, $145. Semen volume, sperm count and concentration, motility. No morphology. This tier isn’t eligible for discounts or insurance.
  • Standard Semen Analysis, $295. Adds morphology, plus lifetime access to the Sperm Insights dashboard and your Sperm Score.
  • Advanced Semen Analysis, $495. Adds sperm DNA fragmentation testing.

Fellow has one option at $199 with shipping both ways included. It covers volume, concentration, count, motility, total motile count and morphology. One thing to know from Fellow’s own site: morphology can come back as “No Result” if your count or motility is low.

So here’s my honest read. If you want the cheapest clinic-grade check of count and motility, Legacy at $145 wins outright. If all you want is morphology and nothing else, Fellow at $199 is cheaper than Legacy’s $295. But the $295 buys you Sperm Insights, and that’s where the comparison stops being close.

Insurance tipped it further. Legacy is in-network with Aetna, Cigna, Emblem and UnitedHealthcare depending on your state, plus Progyny, Carrot, Maven and WIN. Fellow lists Progyny and Maven.

WHAT IS LEGACY SPERM INSIGHTS?

Sperm Insights is Legacy’s results dashboard. It turns a semen analysis into a single Sperm Score, benchmarks you against thousands of Legacy clients your age, and tracks your numbers over time if you test again. It comes with the $295 Standard and $495 Advanced kits, and access is lifetime.

This is the part I’d want everyone to understand before choosing.

With Fellow, you get an emailed link to a results page. It’s a good report. Then it’s over.

With Legacy, your results feed into Sperm Insights. Legacy is openly positioning this alongside Function Health, WHOOP and Oura: sperm as a biomarker for overall health, not just a fertility checkbox. For anyone testing proactively rather than because a clinic told them to, that’s the whole point.

HOW LONG DO LEGACY AND FELLOW SPERM TEST RESULTS TAKE?

Both take about 48 hours from the time the lab receives your sample. Both include overnight shipping, and both ask you to produce your sample Monday through Thursday so it doesn’t sit over a weekend.

Legacy ships its “vault,” an insulated, locked container with transport media. Legacy says its process preserves about 11% more motility than competitor kits and is 30% faster end to end. Legacy is also the only mail-in sperm test reporting estimated initial motility, progressive motility and post-thaw motility.

Fellow uses a preservation solution and a gel pack, and mathematically corrects motility for time in transit.

ARE LEGACY AND FELLOW SPERM TESTS ACCURATE AND LEGIT?

Yes, both run CLIA-certified labs and both have published validation data.

Legacy’s lab is also CLEP-approved, which is the New York State permit, and Legacy has run 75,000+ analyses with a 4.9/5 on Google. The US military trusts them, which still goes a long way with me.

Fellow has ~300,000 tests, 4,000+ clinic partners, 65+ published studies and a 4.6/5 across 15,000+ reviews. If you ARE a clinic, Fellow is built for you. For an individual buying a kit, I’d rather have the insurance coverage and the Insights.

WHAT DOES REDDIT SAY ABOUT LEGACY VS. FELLOW?

Reddit is where the customer service gap shows up. Legacy reviews on Reddit mostly mention accuracy and responsive support. Fellow reviews on Reddit mention a good product but repeated shipping failures and slow, templated support.

Legacy sperm test reviews on Reddit:

  • In r/maleinfertility (Best at home semen analysis test kit?), one couple said Legacy’s results lined up closely with the clinic test their OB ordered afterward. Another described solid communication, a follow-up call with a nurse, and summed it up: “Legacy is amazing highly recommend them.”
  • In r/MtF (Freezing Sperm), a poster’s sample was delayed by FedEx and tested low. Legacy support responded quickly and shipped a free replacement kit. They also called out that samples get split across two storage locations.
  • In r/TryingForABaby (At home sperm analysis), a commenter who’d researched for over a year picked Legacy as the best option available.

Fellow sperm test reviews on Reddit:

  • In r/maleinfertility (At home Male Fertility Test?), a poster said Fellow botched their shipment twice (a spilled sample, then a late delivery), only found out when they chased it themselves, got the same canned reply each time, and lost two months.
  • In r/maleinfertility (My experience with mail in Semen Analysis), one couple had two Fellow samples arrive unreadable because of delivery problems. Another commenter in the same thread was blunter: “Fellow sucks. They screw up all the time.”
  • In r/asktransgender (How many of you banked sperm before HRT?), a poster wanted to redo a weak sample before starting hormones and had to wait about a month because of a shipping backlog on Fellow’s side.

To be fair, Fellow has fans on Reddit too. One r/predaddit user called it one of the best kits out there. But the pattern of shipping failures and slow support came up more than once, and I didn’t find the same pattern for Legacy.

Hope this is helpful!!!!

Legacy, for most people buying a kit themselves. It’s cheaper at the entry level ($145 vs $199), it’s the only at-home option with a Sperm Score dashboard and DNA fragmentation testing, and it’s in-network with major health insurers. Fellow is a strong choice if your clinic already uses it.

How much does a Fellow sperm test cost?

$199 for the Fellow Semen Analysis, shipping both ways included. Fellow’s cryopreservation add-on is $199 plus $240 per year of storage. Prices as of October 2026.

Does Fellow test sperm DNA fragmentation?

No. Fellow’s semen analysis covers volume, count, concentration, motility, total motile count and morphology. Legacy is the only at-home sperm test that includes DNA fragmentation, in its $495 Advanced tier.

Does insurance cover Legacy or Fellow sperm testing?

Legacy is in-network with Aetna, Cigna, Emblem and UnitedHealthcare depending on your state, and works with Progyny, Carrot, Maven, WIN Fertility, Stork Club and the VA. Fellow lists Progyny and Maven as covering its semen analysis. Legacy’s $145 Basic kit is not eligible for insurance.

How long do mail-in sperm test results take?

About 48 hours after the lab receives your sample, for both Legacy and Fellow. Both include overnight shipping, so the whole process usually runs under a week.

Is Legacy’s sperm test accurate?

Legacy’s kit has been clinically validated against in-clinic semen analysis, its lab is CLIA-certified and New York State CLEP-approved, and Reddit users who later tested at a clinic reported results that lined up closely. Expect a small drop in motility with any mail-in kit because of time in transit.
