OVERALL: Fellow wins.
In comparing the Fellow at-home sperm test and the Sapyen at-home sperm test, Fellow was the winner. Sapyen is $50 cheaper and has a bigger menu of add-on tests, but Fellow has what matters most for a diagnostic test: a full validation study published in a peer-reviewed journal, its own CLIA-certified lab, and about 300,000 tests behind it. As of October 2026, Sapyen’s published evidence is one conference abstract.
I want to be fair to Sapyen. Their SPX72 preservation medium is a real idea (72 hours of sample stability instead of a day or two), their $149 Core Semen Analysis is the cheapest full semen analysis I’ve seen with morphology included, and they offer things Fellow doesn’t: DNA fragmentation, antisperm antibodies, DNA methylation, even a semen microbiome test. If those advanced add-ons are what you need, Sapyen is worth a look.
But “most validated at-home test,” which is what Sapyen’s clinician page says, is a claim I couldn’t back up with what they’ve published. That’s the whole ballgame for me.
QUICK FACTS (Oct 2026):
- Fellow’s semen analysis is $199. Sapyen’s Core Semen Analysis is $149.
- Fellow’s validation study was published in Fertility and Sterility (2021) on 104 ejaculates. Sapyen’s published validation is a 2024 ESHRE conference abstract on “over 100” samples.
- Fellow owns and operates its CLIA-certified, COLA-accredited lab. Sapyen sends samples to partner IVF labs.
- Fellow returns results in 1 to 2 business days from lab receipt. Sapyen’s partner clinics quote about 3 days.
- Fellow does not offer DNA fragmentation. Sapyen offers DNA fragmentation ($399), antisperm antibody ($229), DNA methylation ($529) and a combined Complete Analysis ($869).
- Fellow has about 300,000 tests, 4,000+ clinic partners and a 4.6/5 across 15,000+ reviews. Sapyen does not publish a test count.

IS SAPYEN’S SPERM TEST CLINICALLY VALIDATED?
Not in the way Fellow’s is, and this is where the comparison was decided for me.
Fellow published a full validation study in Fertility and Sterility in 2021 (Samplaski et al.). They tested 104 ejaculates, compared one-hour results to delayed results out to 52 hours, and built the correction they use for motility from that data. The whole thing is public, peer-reviewed, and anyone can read the methods. Fellow has since published or presented 65+ studies, including a Journal of Urology paper on post-vasectomy compliance.
Sapyen’s clinician page calls its kit “the most clinically validated at-home male fertility test.” So I went looking for the validation. Here’s what I found as of October 2026:
- One ESHRE 2024 conference abstract (O-015, Human Reproduction supplement). It reports that the SPX72 medium kept motility stable for 72 hours in “over 100” samples. A conference abstract is a half-page summary accepted for a meeting. It is not a full paper and does not go through the same peer review. One of its three authors is Sapyen’s CEO.
- Seven more study titles on the same page (“Breaking the one-hour rule,” “Treatment-decision concordance,” and so on), each with a “Read Publication” label. None had a journal name, a link or a DOI, and I couldn’t find any of them in a journal or conference listing.
- A statistical error in the one abstract that exists. It reports DNA fragmentation of 14.78% in SPX72 samples versus 12.44% in controls and calls the difference “statistically insignificant” with a p-value of 1.15 x 10^-8. A p-value that small means the opposite: the difference was highly significant. Either the number or the conclusion is wrong, and that line got through to publication.
- Competitor prices that don’t match reality. Sapyen’s comparison table lists Fellow’s semen analysis at $229 and Legacy’s DNA fragmentation test at $570. Fellow’s is $199 with free shipping. Legacy’s is $495. Small thing, but it tells you how carefully the page was checked.
None of this means Sapyen’s test is inaccurate. SPX72 may work exactly as described. But “may work” is a different standard from “shown to work in a study you can read,” and for a test people use to decide whether to start IVF, that difference matters. An ASRM 2025 review of eleven at-home and mail-in companies, including Fellow, Legacy and Sapyen, concluded that many fall short on peer-reviewed validation and regulatory transparency. Fellow is the one that doesn’t.
IS FELLOW OR SAPYEN CHEAPER?
Sapyen is cheaper. Its Core Semen Analysis is $149 against Fellow’s $199. Sapyen’s post-vasectomy test is $99 against Fellow’s $149. Both include shipping.
The gap closes once you want more. Sapyen’s DNA fragmentation test is $399 and its Complete Analysis is $869. Fellow doesn’t sell those, so if you need them, Sapyen is the only one of the two that can help. Fellow is covered by Progyny and Maven; Sapyen is available through ARC Fertility and takes HSA/FSA through Truemed.
My honest read: if $50 is the deciding factor and you only want the basics, Sapyen is the cheaper way to get a full semen analysis with morphology. I’d still pay the $50 for the validated one.
WHO ACTUALLY RUNS THE LAB?
Fellow owns its lab. It’s CLIA-certified (05D2177822) and COLA-accredited (31245) in San Leandro, California, and it holds a New York State permit for post-vasectomy testing. Every Fellow sample goes to the same place and the same protocol.
Sapyen doesn’t run a lab. Samples go to partner IVF and diagnostic labs, which Sapyen says are certified. That’s a legitimate model (it’s how a lot of diagnostics work), and testing at an IVF lab is a real selling point. It also means the lab doing your test can vary, and Sapyen’s own validation covers the medium, not the lab network.
HOW LONG DO RESULTS TAKE?
Fellow returns results within 1 to 2 business days of the sample reaching the lab, by secure emailed link. Sapyen’s partner clinics quote about 3 days from receipt, with results in the MySapyen portal. Both include free shipping both ways. Fellow asks you to produce Monday through Thursday; Sapyen’s 72-hour medium gives you more leeway on timing, which is a fair point in its favor.
Hope this is helpful!!!!
FAQ: FELLOW VS. SAPYEN SPERM TESTING
Which is better, Fellow or Sapyen for sperm testing?
Fellow, for anyone who wants a diagnostic-grade result they can take to a doctor. Fellow has a full peer-reviewed validation study, runs its own CLIA-certified lab, and has about 300,000 tests behind it. Sapyen is cheaper ($149 vs $199) and offers advanced add-ons Fellow doesn’t, but its published validation is a single conference abstract.
Is Sapyen legit?
Sapyen is a real company based in Melbourne, Australia, with partnerships at clinics like IVI RMA and Bourn Hall, and its samples are analyzed at partner IVF labs. What it lacks, as of October 2026, is a full peer-reviewed validation paper. Its one published abstract (ESHRE 2024) is co-authored by its CEO and contains a statistical error.
How much does a Sapyen sperm test cost?
$149 for the Core Semen Analysis (count, concentration, motility, morphology, volume, pH). Add-ons: antisperm antibody $229, DNA fragmentation $399, DNA methylation $529, Complete Analysis $869, post-vasectomy test $99. Prices as of October 2026.
How much does a Fellow sperm test cost?
$199 for the Fellow Semen Analysis with shipping both ways included. The Fellow Vasectomy Test is $149. Cryopreservation is $199 plus $240 per year. Prices as of October 2026.
Does Sapyen test DNA fragmentation?
Yes. Sapyen’s DNA Fragmentation Test is $399 and includes the core semen analysis. Fellow does not offer DNA fragmentation testing.
Is Sapyen’s SPX72 medium proven to work?
Sapyen’s ESHRE 2024 abstract reports that SPX72 kept motility stable for 72 hours in over 100 samples. That’s promising, but it’s a conference abstract, not a full peer-reviewed paper, and no independent study of SPX72 has been published that I could find.
How long do Fellow and Sapyen results take?
Fellow returns results within 1 to 2 business days of receiving your sample. Sapyen’s partner clinics quote about 3 days. Both include overnight or express shipping both ways.
